There's No Place Like Home
Northern Home Care is committed to providing the best care possible through personalized attention in the comfort of your own home. Each client is assessed by a professional nurse to determine the appropriate level of care and evaluate risk factors. There is no charge for this assessment, and no obligation.
Following the initial assessment, a physician and case managers develop a care plan in consultation with the patient, family and caregivers. Patient care is monitored daily, and information is communicated to physicians and associated health-care providers to ensure proper levels of care at the highest quality. All payer sources are contacted to discuss the recommended care plan in order to provide for the most cost-effective treatment.

Our focus is not only on the patient, but also on the caregivers: providing them with information, education, and skills training on the aging process and the specific requirements of the patient. Our goal is to empower individuals to make informed decisions about the care of their loved ones.
